Safety I includes long-standing strategies such as behavior-based safety and risk/safety engineering. Safety II promotes human and organizational performance. The safety professional is caught between these two schools of thought, which have some directly opposing ideas, such as their beliefs on the issue of Zero Harm: Safety I states that zero is a non-negotiable goal, while Safety II says that aiming for zero is in fact harmful.

As part of a multi-year intervention at a large mining corporation, Corrie Pitzer developed and deployed a dynamic ‘high reliability system’ with surprising results and success. He will discuss lessons on high reliability theory from the field and the “Deep Safe Organization”.

Following ‘deepdive risk discussions’ with 8000 front line workers in dangerous jobs, discovering over 2000 fatal risks exposure, and after reviewing over 10000 reports of potential Serious Injury, Fatal or Catastrophic (SIFC) risks, a new ‘model’ for risk emerged: The Random, Chaotic, Complex risks that lurks in the depth of all organizations – waiting to be discovered, or to explode in events.


Corrie gained new insights from what he calls the” 3rd Layer of Risk”, where not only the Black Swans live, but also the Purple Dragons, the Yellow Elephants and the mysterious White Slugs.


Corrie will discuss “8 missing links” in safety, glaringly obvious, but we fail to ‘see’ them, or manage them.  And he will unveil the new formula: Ef=mc3
