To achieve this, we must prevent and predict risks. Yet, despite extensive efforts, success remains elusive. While we can identify the risks and understand how they occur\u2014such as high-energy work\u2014we cannot reliably predict when or where they will happen. This unpredictability highlights two challenges:\n<ol>\n<li>Traditional systems for identifying serious injury and fatality (SIF) risks rely on hindsight, analyzing past accidents or near misses.<\/li>\n<li>Static identification tools like audits, inspections, and risk registers fail to capture dynamic, evolving risks as they emerge.<\/li><\/ol>\n\n\n\n<p class=\"\">This presentation introduces a new approach: &#8220;randomized readiness for risk\u201d response, redefining safety the safety approach.
