As part of a multi-year intervention at a large mining corporation, Corrie Pitzer developed and deployed a dynamic \u2018high reliability system\u2019 with surprising results and success. He will discuss lessons on high reliability theory from the field and the \u201cDeep Safe Organization\u201d.<\/p>\n\n\n\n

Following \u2018deepdive risk discussions\u2019 with 8000 front line workers in dangerous jobs, discovering over 2000 fatal risks exposure, and after reviewing over 10000 reports of potential Serious Injury, Fatal or Catastrophic (SIFC) risks, a new \u2018model\u2019 for risk emerged: The Random, Chaotic, Complex risks that lurks in the depth of all organizations \u2013 waiting to be discovered, or to explode in events.<\/p>\n\n\n\n


Corrie gained new insights from what he calls the\u201d 3rd Layer of Risk\u201d, where not only the Black Swans live, but also the Purple Dragons, the Yellow Elephants and the mysterious White Slugs.<\/p>\n\n\n\n
