When \u2018error\u2019 occurs, complex iterations of perceptions, intentions, interpretations were in play that could have had a different outcome.<\/p>\n<p><strong>Systems and culture drive behavior<\/strong><br \/>\nHuman actions are largely a responseto the processes, pressures, demands, requirements as perceived by individuals and teams, and more often than not, culture will trump the system.<\/p>\n<p><strong>Work requires both technical and risk competence<\/strong><br \/>\nCompetence has two components, i.e. technical, that is the result of the skills, training and task procedures, while risk competence refers to the skills to identify, judge and deal with risks, commonly referred to as situational awareness.<\/p>\n<p><strong>All \u2018events\u2019 are complex and dynamic<\/strong><br \/>\nThe common accident, slips trips and falls, has common and linear causes, largely unpreventable, while uncommon events have complex, random and dynamic \u2018origins\u2019 (not causes).<\/p>\n<p><strong>Safe work is the readiness to respond to risk<\/strong><br \/>\nThe absence of accidents is not a valid measurement of the existence of \u2018safety\u2019 in an organization, as significant risk exposures can exist, but no events recorded.
