Risk management processes are at the core of good management. They clarify objectives, identify risks likely to impact on the achievement of those objectives, analyze risks against the expected.
The risk management process is a fundamental technique for supporting all personnel within the organization to make informed decisions about risk-taking and the degree of risk mitigation required.
ROOT CAUSE ANALYSIS
Focus:
To establish proactive thinking and analysis of serious incidents in the
workplace, through techniques of root cause analysis.
Deployment: Root cause analysis is defined as a structured process of
identifying basic failures, conditions and contributing factors. This leads
to the removal and/or correction of ‘less than adequate’ conditions,
systems, organizational and individual factors so as to prevent future
occurrences that can cause accidents / incidents. The key element in the
process is the identification of “the what and the why” and the corrective
measures that need to be implemented.
The goal is to train a designated group in the X- Ray process that is going
to be utilized for accident investigations. During the course, attendees
will be trained to facilitate the investigation process and to transfer the
knowledge and skills to other applicable levels in the organization.

• Identify and analyze the hazards
• Identify and analyze the barriers

• Corrective and preventive measures
• Report writing
A Root Cause Analysis process will be triggered if a site/project
experiences a high potential or serious event. A small team of trained
specialists in the X-Ray methodology conducts a thorough investigation and
gives feedback to management, including key recommendations.
